Instructions
You are the newly hired junior associate in a not-for-profit healthcare system that is exploring opportunities for a two-tiered system of care. Specifically, the healthcare system is committed to maintaining a not-for-profit, community-oriented population-health level of care. They are also investigating an opportunity to deliver a more personalized and individual model of care. Aware that you have taken a course in population health, your supervisor asks you to prepare a brief graphic presentation for your colleagues on the differences, similarities, and opportunities for synergy using the population health and the medical models of care.
Construct a table of model characteristics with population health at one end and the medical model on the other. Place opportunities for synergy (shared characteristics) in the middle of the table as shown. For example, one of the distinguishing characteristics of population health is a community orientation.
